A question often asked from our community is “How do I get my loved one in to a nursing home for long term care?”
The first 2 important steps towards consideration for a long term care admission are:
1. Obtaining a PRI (Patient Review Instrument) & Screen:
a. Contact your loved one’s primary care physician and/or contact your local visiting nurses such as The Eddy to set up an appointment for a PRI certified nurse to come to your home and fill out this documentation. A PRI simply gives the nursing home or assisted living a snapshot of level of care needed such as how much assistance is needed for mobility, transfers, dressing & bathing, etc. This documentation also addresses insurance, medications, precautions, allergies, etc so the healthcare facility can determine if they can clinically care for the person.
b. PRI is valid for only 30 days
If the loved one is not in the hospital or healthcare facility, there may by a fee attached to obtaining a PRI.
2. Long Term Care Payor: Chronic care is paid for by:
a. Medicaid
b. Private Funds
c. Long Term Care Insurance Policy
d. You may be asked to fill out a universal application for the facility to review funds.

A quick ditty about Greene Meadows Nursing & Rehabilitation Center can be summed up like this:
We were fully renovated when the new ownership purchased the facility in 2016.
We believe in family values and providing good care to the community. We would like to be considered your “extended family”.
We can care for up to 120 people at Greene Meadows for short term rehabilitation, long term care and memory care (Alzheimer’s Dementia) with appropriate activities and program for each. Along with music therapy, pet therapy and religious services, Greene Meadows prides ourselves as being top of the line for short term rehabilitation including Physical Therapy, Occupational Therapy and Speech Therapy up to 7 days a week!
A few of the many amenities we offer are:
- On-campus dialysis (which means no costly transportation fees. As a matter of fact, no need for outside transport at all since we are in the same building!)
- In house smart car for safe car transfer training for the patient and family!
- Social services to assist in continuum of care and discharge planning
- Vohra Wound Care Center of Excellence-Greene Meadows is the only facility in New York who has received this award 2 years in a row!
